Recently, I got a movie from Netflix. This is the DVD that arrived:
At a glance, it appears to be a copied DVD. As readers of the book or viewers of the movie will understand, however, this is custom artwork which references the story. The logos on the disc ought to be a tip-off, but it seems that enough complaints were raised (or anticipated) for Netflix to put a special note on the sleeve:

With this note, Netflix can handle the matter pretty easily for their customers. But one has to wonder just how many people have tried returning their legitimately-purchased DVDs to retailers, after being confronted by this disc. Sure enough, it appears that when the movie was released, confusion reigned.
